Confirmation Name Report For the name you wish to take as part of your reception of the Sacrament of Confirmation, You may:
Choose to take again your baptismal name (it must be a name of a recognized saint of the Church)
OR
You may choose a new name but It must be the name of a recognized saint of the Church or a title of Jesus or Mary. No matter which one you choose, you are required to submit a report about your saint and your report must include the following:
Give the name of your saint, the saint’s feast day and if the saint has been named a patron for a particular group of people or cause.
Give a couple of sentences about the saint’s background i.e. where the saint lived, saint’s family, saint’s occupation/ministry.
Write three to five sentences about the saint’s life and/or death that showed the virtues this saint practiced.
Tell how this saint is a model for Christians today.
Tell what aspect of this person’s Christian life attracted you? Or if you chose the saint’s name because of someone you admire, tell why you admire or admired this person.
Finally list the resource(s) you used to gather your information.
Instructions: Your report must be written in ink, neatly or typed (12 to 14 font size) You should use complete sentences, correct grammar and spelling. Your report should have a title – the name of the saint would be OK. Your report must have a cover with the name of the saint, a picture or symbol that represents the saint and your name.
